Notes:

1.  Drug 1 thru Drug 15 holds the name of each drug tested for. If Alcohol was tested, it will appear 
     after the last drug. I.E. in a 10 panel, alcohol would appear in Drug 11. 
2.  Result 1 thru Result 15 holds the result for each drug tested. 
3.  Screen Cutoff 1 thru Screen Cutoff 15 holds the screen cutoffs for each drug tested.
4.  GCMS Cutoff 1 thru GCMS Cutoff 15 holds the gcms cutoffs for each drug tested.
5.  Dr. Signature will only contain a name when the test is negative.
6.  Creatnine will contain a 'Y' if the creatnine level was tested and found to be low.
7.  Comment id is not currently used and should always be blank.
